Diabetes Educator

Fairview Health Services is seeking a Certified Diabetes Educator to join our Endocrinology Clinic team at M Health Fairview Clinics & Surgery Center. This is a 0.8 FTE on the Day shift, 7:30am-4:00pm or 8:00am-4:30pm. The educator will manage patients with chronic disease goals, provide patient education on diabetes management, collaborates with physicians, dieticians, and the care team, and helps develop related policies and programs. Requires CDE certification and relevant experience. #J-18808-Ljbffr
