Fairview Health Services is seeking a Certified Diabetes Educator to join our Endocrinology Clinic team at M Health Fairview Clinics & Surgery Center. This is a 0.8 FTE on the Day shift, 7:30am-4:00pm or 8:00am-4:30pm. The educator will manage patients with chronic disease goals, provide patient education on diabetes management, collaborates with physicians, dieticians, and the care team, and helps develop related policies and programs.
