Humana is seeking a Registered Nurse Care Manager in Ladysmith to coordinate and manage care for members with complex or chronic conditions. In this role, you will complete telephonic and occasional in-person assessments, develop individualized care plans, and collaborate with physicians, social workers, and community resources to close care gaps and improve outcomes. You will educate members and caregivers, monitor progress using data and EHR tools, and advocate for services that reduce hospitalizations while supporting Humana’s member-focused, inclusive culture.
