As a compassionate and dedicated Licensed Practical Nurse (LPN), you will play a vital role in delivering exceptional care to our residents.The Licensed Practical Nurse (LPN) provides direct nursing care to residents in accordance with physician orders, facility policies, state and federal regulations, and accepted standards of nursing practice. The LPN works under the supervision of a Registered Nurse and collaborates with the interdisciplinary team to ensure quality resident care and services.Responsibilities:Administer medications and treatments as prescribed by physicians.Monitor residents' health status and report significant changes to the physician and supervising nurse.Provide wound care and other clinical interventionsPerform nursing assessments within scope of practice.Ensure accurate and timely documentation is informative and in a descriptive manner.Admit, discharge, and transfer residents.Supervise CNAs to ensure adherence to care standards in accordance with current rules, regulations, and guidelinesMaintain infection prevention and control practicesSupport implementation and updates of individualized care plans for each resident.Safeguard resident rights, privacy, and dignity (HIPAA compliance)Notify the residents' attending physicians and next-of-kin when there is a change in the residents' conditions or if the residents are involved in an incident or accidentAttend and participate in all center in-service programs as requiredQualifications:Must hold a current and valid LPN license in the state employedLong-term care/post-acute care setting preferredCompletion of post-secondary educationCapable of maintaining regular attendance (i.e., weekend shifts and overtime) as requiredWhat We Offer:Opportunities for career advancementA work environment that values diversity and inclusionShift DifferentialContinuing Education CreditsJob Type: Full-timeAvailable Schedules:7AM-7PM7PM-7AMLicense/Certification:BLS Certification (Preferred)LPN (Required)Physical Setting:Long-term careRehabilitation centerWeekly Schedule:3x12 alternating4X12 alternatingHow to Apply:If you are a motivated and compassionate LPN looking for a new challenge, we encourage you to apply to this exciting opportunity.
