SummitWest Care is seeking a compassionate Speech-Language Pathologist (SLP) to provide one-on-one care throughout Mesa County on an as-needed basis. One-on-one patient care with time to assess, educate, and build meaningful therapeutic relationships Provide therapy where it matters most in the patient's actual living environment Improve quality of life through communication, cognitive, and swallowing interventions Flexible PRN scheduling that works around your existing commitments Clinical autonomy with the support of an experienced interdisciplinary team Strong collaboration with nurses, therapists, physicians, caregivers, and office staff Mission-driven nonprofit organization focused on patient outcomes rather than productivity metrics Responsibilities Perform speech-language pathology evaluations, reassessments, recertifications, and discharges Assess and treat speech, language, cognitive-communication, and swallowing disorders Develop and implement individualized treatment plans based on patient goals and clinical needs Educate patients, caregivers, and family members on treatment recommendations and home exercises Collaborate with physicians, nurses, therapists, caregivers, and other members of the care team Monitor patient progress and adjust treatment plans as appropriate Recommend compensatory strategies, adaptive techniques, and safety interventions when needed Complete accurate and timely clinical documentation Participate in interdisciplinary care planning to support optimal patient outcomes Qualifications Current Colorado Speech-Language Pathologist (SLP) license in good standing Master's Degree from an accredited Speech-Language Pathology program Certificate of Clinical Competence (CCC-SLP) preferred Strong clinical assessment, communication, and documentation skills Experience with adult and geriatric populations preferred Ability to work independently in a home health environment Excellent time-management and organizational skills Valid driver's license, reliable transportation, and auto insurance Home health experience is a bonus, but not required! PRN Position Information This position is referral-based and patient needs may fluctuate throughout the year. At times there may be consistent visits available, while other periods may involve little or no active caseload.
