Key Responsibilities

• Provide physical therapy services to patients within a home health setting across the Skagit and North Snohomish coverage area
• Conduct evaluations, develop individualized treatment plans, and deliver skilled PT interventions in the home environment
• Document patient care accurately and in a timely manner using Homecare Homebase (HCHB)
• Coordinate with the interdisciplinary care team to ensure continuity and quality of patient care

Required Qualifications

• Active Physical Therapist (PT) license in the state of Washington
• Homecare Homebase (HCHB) experience required
• Prior experience in a home health setting
• Valid driver's license and reliable transportation for home visits within the coverage area

Preferred Experience / Skills

• Familiarity with the Skagit and North Snohomish County regions
• Strong organizational and time-management skills for managing a home-based caseload
• Experience working independently in a field-based therapy role
