Job Opportunity

Position Overview

This position requires a skilled professional to provide therapy services in a home health setting. The selected candidate will conduct home visits, participate in periodic office meetings, and must have reliable transportation along with the necessary licensure and insurance.

Schedule and Expectations

• Shift: Day 5x8-Hour (08:00 - 17:00)
• Average Number of Patients per Day: 5-6
• Weekly Points Requirement: 30 points per week, with each visit and activity, including documentation and travel, assigned different point values.
• Expected Mileage: Approx. 250 miles per week, reimbursed at the IRS standard rate.
• Contract Duration: 13 weeks
• Availability of Guaranteed Hours: No guaranteed hours; flexibility is required depending on patient census.

Travel Requirements

Candidates must be willing to travel up to 75 miles from their assigned home clinic. The commute to and from the first and last patient is not billable. Local candidates are welcome to apply.

Key Responsibilities

• Conduct home health visits, including Start of Care and Follow-Up assessments.
• Complete all documentation within 24 hours of patient appointments to ensure seamless patient care and billing.
• Accept all scheduled patients, regardless of appointment location, to meet productivity expectations.
• Attend weekly office meetings and participate in additional training as required.

Qualifications

• Experience: Minimum of 1 year of adult home health experience.
• Certifications: Current CPR and BLS certifications are required.
• Licensure: Must have an active license at the time of application. Pending licenses will not be accepted unless they require only a few days to obtain.
• Software Proficiency: Familiarity with HomeCare HomeBase (HCHB) EMR system and OASIS within the last 6 months is required.

Geographic Coverage

The role will cover areas including Norton, Attleboro, Mansfield, Plainville, North Attleboro, Wrentham, Foxboro, and Seekonk.

Additional Information

Weekend requirements may vary based on location and position. On-call duties may also be part of the job; please discuss specifics during the interview process.
