As Circadia’s on-site NP, you’ll see patients first, act on predictive health signals in real time, and coach facility teams to turn data into earlier interventions—reducing preventable readmissions. You’ll blend direct patient care with tech-to-bedside integration through hands-on training, live demos, and shadowed assessments. This is a full-time, in field role requiring Monday–Friday presence at our partner facilities in Central New Jersey. Direct patient care: Conduct initial and follow-up assessments on flagged residents; initiate/adjust plans of care; coordinate with facility providers. Train and coach staff at the point of care, standardize use of Circadia into clinical workflows. Data into action : Interpret trend-based insights, prioritize high-risk patients, and drive timely interventions to prevent decline. 30+/day where appropriate), focusing on new admits, facility-flagged patients, and Circadia alerts. Quality & product feedback : Surface field insights to improve workflows and tech integration in a clinical setting Cover sites within a 30–40-mile radius; for sites outside of that radius, mileage and lodging covered per policy. Visits: Target 30 patient visits/day Adoption & impact: Demonstrated workflow adherence at facilities; Certified and licensed Nurse Practitioner in NJ; ANCC (or equivalent) in Adult/Family/Geriatric/Acute.

• 2+ years NP experience with complex adult populations; comfortable traveling locally.
• English required

Google Suite, multiple EMRs, Slack, AI transcription/tools. Spanish speaking is preferred. \As a full-time Nurse Practitioner, you will be employed by Circadia Health, Inc. The anticipated annual base salary range for this full-time position is $140,000 - $180,000 .
