Registered Nurse (RN) Home Health Admissions (SOC) – Full‑time position in North Chelmsford, Massachusetts. Responsibilities Complete OASIS Start of Care (SOC) assessments for adult and geriatric patients. Accurately document observations, interventions, and evaluations pertaining to client care management using a touch pad tablet. Qualifications Current RN license in Massachusetts.
