About the Role

Platinum Strength is a growing Chicago-based physical therapy and senior wellness company preparing to launch programs across multiple senior apartment communities.

We are looking for a licensed Physical Therapist who enjoys working with older adults and wants a flexible, community-based role with the opportunity to grow alongside a new practice.

This is not a traditional clinic position. Care will be provided onsite at senior apartment communities, with a focus on strength, balance, mobility, fall prevention, pain reduction, and helping residents remain independent.

For the right person, this role has the potential to expand into a larger clinical and leadership position as the company grows.

Responsibilities

• Perform physical therapy evaluations and develop individualized plans of care
• Provide skilled outpatient physical therapy to residents onsite at senior apartment communities
• Treat balance deficits, weakness, gait limitations, mobility issues, pain, and other functional impairments
• Lead or participate in senior strength, balance, mobility, and fall-prevention programming
• Complete accurate and timely clinical documentation
• Track patient progress and adjust treatment plans as appropriate
• Communicate effectively with patients, families, physicians, and the broader care team when needed
• Help shape clinical workflows as the practice grows

Qualifications

• Active Illinois Physical Therapist license
• Experience working with older adults preferred
• Outpatient, geriatric, home-based, or senior-living experience is a plus
• Comfortable working independently in a community-based setting
• Strong communication and documentation skills
• Medicare Part B experience preferred but not required
• Interest in joining a growing practice and potentially taking on additional responsibility over time
