Plan, administer, and document personalized physical therapy treatments in patients' homes to improve physical function. Assess patients' strengths, develop rehabilitation programs aligned with physician plans, and monitor progress. Provide clear instructions and oversee performance of PTAs and Home Health Aides. Communicate patient progress to the care team, physicians, and families, and complete all required evaluations and summaries.
