These positions will provide non-clinical and case management services to promote the self-management of chronic diseases to members with special health care needs. Applicants with behavioral/mental health/psychiatric, pediatric, and case/care management experience are especially encouraged to apply. Required:

• Experience in case management, human services, or public health
• Experience in provisioning of referral and follow-up services
• Experience with completing psychosocial assessments
