Job Title: Acute Care RN Case Manager – Utilization Review Profession: Mid-Revenue Cycle Solutions Specialty: Acute Care RN Case Manager Duration: 13 weeks Shift: 0800-1630 Hours per Shift: 8 Experience: Minimum 5 years License: Active RN License Certifications: Relevant certifications in case management and utilization review Must-Have: - Strong knowledge of Medicare, Medicaid, HMO, and private insurance coverage criteria - Experience in routine case management and discharge planning - Ability to communicate effectively with patients, families, and healthcare team members Description: This position is responsible for routine case management and discharge planning. The role includes swing bed coordination and utilization review duties. Evaluate and screen potential admissions to the swing bed program. Knowledge of criteria for insurance coverage is essential. Initiate ongoing communication with residents and their families to assess discharge needs. Actively participate in daily rounds with bedside staff, physicians, and ancillary team members. Communicate with physicians to ascertain their plans for timely discharge. Document updates and discharge planning as an ongoing review process. Manage and collaborate with the healthcare team on swing bed placement. Complete MDS documentation as necessary. Assist with obtaining referrals and prior authorizations for Home Health Care and appointments. Educate physicians and staff regarding appropriate levels of care, utilization, and required documentation. Run Interqual for each inpatient and outpatient observation patient admitted to the floor.
