Pride Health is hiring an RN Hospice Case Manager to support our client’s medical facility based in Fort Worth, TX . This is a 13-week assignment with a contract-to-hire opportunity and a great way to start working with a top-tier healthcare organization!
Job Title: RN Hospice Case Manager
Location: Fort Worth, TX
Weekly travel blended pay: $1,950/week - $2,160.00/week
Schedule: Monday–Friday, 8:00 AM – 5:00 PM
Start date: 06/15/2026
Primary Responsibilities:
Assess patients' physical, psychosocial, spiritual, developmental, environmental, and safety needs.
Develop, implement, and update individualized care plans.
Manage pain and symptom control to ensure patient comfort.
Provide end-of-life and comfort-focused care.
Support patients and families through the dying process and grief.
Coordinate care and services with the interdisciplinary team (IDT).
Educate patients, caregivers, and families on medications, treatments, and care plans.
Maintain accurate and compliant electronic medical records.
Collaborate with physicians, social workers, chaplains, and other healthcare professionals.
Deliver holistic, family-centered hospice care while respecting cultural and spiritual beliefs.

End-of-life support
HCHB experience is required.
