Youth & Family Prevention Coordinator

BUILD, Inc. in Chicago is seeking a Prevention Care Coordinator for the Violence Prevention Villages (VPV) project. The role involves assessing youth aged 3–15 and their households, coordinating services with a team of direct service providers, and leveraging partner agencies to address needs. You will help develop care plans, monitor progress, and document outcomes.
