RN Case Manager - Home Health

Location Bradford, VT / Randolph, VT / Springfield, VT / White River Junction, VT / Lebanon, NH

This is a 13-week contract position.

Days, Full Time

As an RN Case Manager at Visiting Nurse and Hospice for Vermont and New Hampshire, you will play a vital role in delivering superior nursing, rehabilitation, hospice, and personal care services. You will have the ability to develop close relationships with patients, providing individualized care and support as part of a cohesive patient-care team.

Responsibilities

• Develop a care plan in collaboration with the patient's physician, patient, and caregivers upon admission.
• Periodically review and recertify the patient care plan as appropriate.
• Coordinate the patient's care plan through interdisciplinary communication and case conferences.
• Provide ongoing assessment of the patient's clinical, functional, and emotional status.
• Document evaluations, treatment goals, and objectives.
• Teach self-care techniques to patients and caregivers as necessary.
• Provide instructions about medications, diet, and health counseling as ordered by the physician.

Required Experience / Certifications / Licensure

• Must be a licensed Registered Nurse in Vermont and New Hampshire or hold an unencumbered compact license.
• Graduate of an accredited school of nursing.
• 2 years of experience in Home Health.
