Responsibilities
- Provide direct nursing care to residents in a long-term care setting under supervision, including assessment and monitoring of patient status
- Administer medications and treatments as prescribed; document with accuracy and report adverse effects or changes promptly to the supervising RN
- Assist residents with activities of daily living (ADLs) and provide comfort and support to residents with dementia or cognitive impairment
- Monitor vital signs, perform routine wound care, and manage basic nursing tasks aligned with care plans
- Maintain safe and sanitary environments; observe infection control practices and fall prevention strategies
- Communicate effectively with residents, families, and the care team; participate in care planning and care transition activities
- Must be willing to float—no exceptions, to support staffing needs across units as required

Requirements
- Active New York Licensed Practical Nurse (LPN) license
- Basic Life Support (BLS) certification
- Minimum 1+ year of current experience in long-term care or geriatric nursing
- Ability to work in a long-term care setting and handle dementia-related care
- Willingness to float to other units as needed to meet patient care requirements
