Physical Therapist - Per Diem

Physical Therapist In Home Health Role and Responsibilities: Conduct patient assessments to determine functional status, mobility, pain, and medical needs. Develop and implement personalized treatment plans to improve patient well-being, monitor progress, and adjust care as needed. Collaborate with healthcare teams, provide patient and caregiver education, and document visits and outcomes according to policies. Deliver care in diverse home environments, ensuring compliance with state laws and standards to maximize patient function and comfort. Qualifications and Skills: Current state licensure in Physical Therapy Valid driver's license Vehicle insurance Access to reliable transportation CPR certification preferred or obtainable within 90 days Strong communication, assessment, and teamwork skills required Compensation: Competitive pay includes per visit and hourly rates, based on productivity and performance.
