Resident Care Coordinator

Position Summary The Resident Care Coordinator (RMA) is responsible for coordinating and overseeing resident care services in an assisted living community while supporting staffing operations and participating in the on-call rotation. This role ensures that residents receive high-quality, person-centered care in accordance with state regulations and company standards. The Coordinator works collaboratively with leadership and scheduling functions to maintain appropriate staffing levels, respond to real-time care needs, and ensure safe and effective service delivery. This position serves as a liaison between residents, families, staff, and management. Key Responsibilities Coordinate and oversee daily resident care services to ensure care plans are followed and resident needs are met. Conduct resident assessments and assist with development and updates of individualized service plans. Monitor residents for changes in condition and communicate updates to families, physicians, and leadership. Ensure accurate and timely documentation of care, incidents, and health status changes. Assist with medication administration within RMA scope and state regulations. Promote resident dignity, independence, and overall quality of life. Staff Supervision & Support Provide daily oversight and direction to caregivers and medication aides. Assign duties based on resident needs, acuity, and staffing levels. Support orientation, training, and ongoing development of care staff. Monitor staff performance and provide coaching to ensure quality care delivery. Serve as a resource to staff for clinical and operational guidance during shifts. Staffing & Scheduling Coordination (Shared Responsibility) Collaborate with leadership and/or designated scheduler to develop and maintain staff schedules. Assist in ensuring adequate staffing coverage for all shifts based on resident acuity and census. Manage and respond to call‑offs, schedule adjustments, and last‑minute coverage needs. Communicate staffing needs and trends to leadership to support workforce planning. Participate in staffing decisions to ensure safe and efficient care delivery. On-Call Responsibilities Participate in an on-call rotation to support staffing and resident care needs outside of normal business hours. Respond to urgent staffing shortages, resident concerns, and operational issues. Provide guidance and decision‑making support to staff during after‑hours situations. Escalate critical issues to leadership as appropriate. Compliance & Quality Assurance Ensure compliance with applicable state regulations, including Virginia assisted living standards. Support medication management oversight per RMA scope and supervision requirements. Assist with audits, surveys, and quality assurance initiatives. Maintain accurate and up‑to‑date resident records and service plans. Promote infection control, safety, and risk management practices. Communication & Family Engagement Serve as a key point of contact for residents and families regarding care services. Communicate effectively with interdisciplinary team members to ensure continuity of care. Participate in care plan meetings and provide updates on resident status and needs. Qualifications Active Registered Medication Aide (RMA) certification in the Commonwealth of Virginia (required). Previous experience in assisted living, long‑term care, or senior housing (preferred). Supervisory or leadership experience preferred. Knowledge of resident care practices and assisted living regulations. Strong organizational, time management, and problem‑solving skills. Ability to manage multiple priorities in a fast‑paced environment. Excellent communication and interpersonal skills. Work Environment & Requirements Ability to stand, walk, bend, and assist residents as needed. Must be available for flexible scheduling, including evenings, weekends, and on‑call rotation. Ability to respond to urgent situations in a timely manner. Core Competencies Resident‑Centered Care Leadership & Team Coordination Staffing & Scheduling Awareness Clinical Observation & Attention to Detail Communication & Relationship Management Problem‑Solving & Decision‑Making Certified Medication Aide CMA Responsibilities Administers medications as ordered by a physician under the supervision of a licensed nurse, in accordance with any state and federal regulations and consistent with community policy. Must demonstrate knowledge of the "five rights" of medication administration. Administers and records all medication given to residents based upon their specific chart. May take and record vital signs of residents as assigned or as may be required by certain medications. Records the administration of medications and/or treatments appropriately, utilizing the computer system to enter and reference resident information. Observes and reports any reactions and/or side effects of medications to a licensed nurse immediately. Administers PRN medications in accordance with state and federal regulations and consistent with facility policy. Ensures antiseptic techniques and good hand washing practices for infection control, as well as utilizing universal precautions. Reports and records any changes observed in condition or behavior of residents and unusual incidents. Submits incident/accident reports to supervisor or Administrator within twenty‑four (24) hours after their occurrence. Communicates with medical staff, nursing service and other department supervisors. Supervises care staff in absence of nurses. Maintains, cleans and organizes the Medication Room. Maintains liaison with other department supervisors to adequately plan for resident activities. Attends all required training, in‑service and staff meetings. May be subject to handling and exposure to hazardous chemicals. May be subject to exposure to infectious waste, diseases, conditions, etc., including TB and AIDS and Hepatitis B viruses. Maintains resident confidentiality, staff confidentiality; treats staff and residents with kindness, dignity and respect. Complies with Resident Rights rules. Certified Medication Aide CMA Requirements High School Diploma or General Education Degree; or one to three months related experience and/or training; or equivalent combination of education and experience. Certified Medication Aide certification required.
