LPN Med Care Manager

Role Overview The Licensed Practical Nurse (LPN) Medication Nurse provides essential nursing support by administering medications, monitoring resident health, and ensuring accurate clinical documentation in a senior living environment. Key Responsibilities Administer medications and treatments according to physician orders, following the Six Rights of Medication Administration Accurately document in MAR/TAR in real-time Monitor symptoms, report adverse reactions, and observe physical, emotional, or cognitive changes Transcribe physician orders and support care plan updates Maintain medication carts, conduct narcotic counts, and ensure compliance with safety standards Conduct Service & Health Updates (SHUs) and support resident safety through clinical precision Qualifications & Skills Active LPN license in good standing CPR and First Aid certification Strong assessment and documentation abilities Experience in senior care or medication administration preferred Key Duties Deliver safe, accurate medication administration, monitor health changes, support care plan updates, and uphold compliance standards to enhance resident well-being. #J-18808-Ljbffr
