CareCore at Willowood is seeking a compassionate and dedicated PRN Physical Therapist (PT) to join our exceptional in-house rehabilitation team. This is an excellent opportunity for a therapist looking for flexible scheduling while making a meaningful impact in the lives of residents in a skilled nursing and long-term care setting. Position Highlights PRN (as-needed) schedule Flexible weekday and weekend opportunities In-house rehabilitation program Skilled Nursing & Long-Term Care setting Collaborative interdisciplinary team Opportunity to pick up additional hours as available Responsibilities Perform comprehensive physical therapy evaluations Develop and implement individualized treatment plans Provide evidence-based therapy services to help residents improve mobility, strength, balance, and functional independence Collaborate with physicians, nursing staff, therapy team members, residents, and families Complete accurate and timely documentation using Net Health Utilize PointClickCare (PCC) as part of the interdisciplinary care process Maintain compliance with all federal, state, and company regulations Qualifications Current Ohio Physical Therapist (PT) license in good standing Graduate of an accredited Physical Therapy program Skilled nursing or long-term care experience preferred, but not required Excellent communication, documentation, and clinical decision-making skills Experience with Net Health and PointClickCare (PCC) is preferred Why CareCore at Willowood?
