Job Description:\n\nADULT DAY HEALTH SERVICES – JOB DESCRIPTION\nDepartment: Adult Day Health / Direct Care ServicesEmployment Status: Full-Time / Part-Time / PRNClassification: Direct Care Staff\n\nPOSITION SUMMARY\nThe Certified Nursing Assistant (CNA) provides direct personal care, assistance with activities of daily living, observation, supervision, and supportive services to participants attending the Adult Day Health Center.\nThe CNA works under the direction and supervision of licensed nursing personnel and center management and provides services according to each participant's Individual Plan of Care, center policies and procedures, assigned duties, training and competency, and applicable requirements.\nThe CNA is responsible for promoting participant health, safety, dignity, independence, comfort, choice, and participation while promptly reporting changes in participant condition to the Registered Nurse or other appropriate licensed nursing personnel.\nThe CNA shall not independently perform nursing assessments, diagnose medical conditions, develop nursing plans of care, administer medications, or perform other functions reserved for licensed nursing personnel unless specifically authorized by applicable law and the individual's credentials.\n\nESSENTIAL DUTIES AND RESPONSIBILITIES\n\n Activities of Daily Living\n\nProvide or assist participants with activities of daily living according to their Individual Plan of Care, including:\n\nToileting and continence care.\nPersonal hygiene.\nHandwashing.\nOral care.\nGrooming.\nDressing.\nEating and drinking.\nMobility and ambulation.\nTransfers.\nPositioning.\nWheelchair assistance.\nRest periods.\nOther assigned personal-care activities.\n\nEncourage participants to perform as much of their own care as safely possible to promote independence and maintain functional abilities.\n\n Participant Supervision and Safety\n\nThe CNA shall:\n\nProvide appropriate supervision of assigned participants.\nMaintain awareness of participant locations and activities.\nProtect participants from injury and unsafe conditions.\nFollow participant-specific safety precautions.\nObserve participants identified as fall, wandering, elopement, choking, aspiration, seizure, or other safety risks.\nImmediately report unsafe conditions.\nNever leave participants unattended when supervision is required.\nFollow the participant's Individual Plan of Care and staff instructions.\nAssist the center in maintaining appropriate direct-care staffing coverage.\n\nObservation and Reporting\n\nThe CNA plays an important role in recognizing changes in participant condition.\nThe CNA shall observe participants for changes involving:\n\nMental status.\nAlertness.\nBehavior.\nMobility.\nBalance.\nSkin condition.\nEating and drinking.\nToileting.\nBowel or bladder patterns.\nPain or discomfort.\nBreathing.\nWeakness or fatigue.\nSwelling.\nFalls or injuries.\nConfusion.\nFunctional ability.\nOther unusual signs or symptoms.\n\nThe CNA shall promptly report significant observations or changes to the RN or other appropriate licensed nursing personnel.\nThe CNA shall not independently diagnose or treat a participant's medical condition.\n\n Vital Signs and Routine Measurements\n\nWhen trained, competent, assigned, and permitted by center policy, the CNA may obtain and document routine measurements such as:\n\nTemperature.\nPulse.\nRespirations.\nBlood pressure.\nWeight.\nOther routine measurements authorized within the CNA's role.\n\nAbnormal findings or significant changes must be promptly reported to licensed nursing personnel.\nThe CNA shall not independently interpret findings or make clinical treatment decisions.\n\n Mobility, Transfers and Positioning\n\nThe CNA shall:\n\nAssist participants with walking and mobility.\nAssist with wheelchair use.\nAssist participants with transfers according to established care instructions.\nUse gait belts or other approved equipment when trained and required.\nFollow participant-specific transfer precautions.\nAssist with repositioning when indicated.\nEncourage safe mobility and independence.\nUse proper body mechanics.\nRequest additional staff assistance when a participant requires a two-person transfer or other additional support.\nNever attempt an unsafe one-person lift or transfer.\n\nAny significant decline in mobility or ability to transfer safely must be reported to licensed nursing personnel.\n\n Meals, Snacks and Hydration\n\nThe CNA shall:\n\nAssist participants with meals and snacks.\nSet up meals when needed.\nAssist with feeding when assigned and trained.\nEncourage appropriate fluid intake.\nFollow participant-specific dietary instructions.\nFollow identified swallowing and aspiration precautions.\nObserve participants for difficulty chewing or swallowing.\nImmediately respond to and report coughing, choking, difficulty swallowing, or other feeding concerns.\nDocument intake when required by the participant's plan.\nReport poor food or fluid intake to licensed nursing personnel.\n\n Toileting and Continence Care\n\nThe CNA shall:\n\nAssist participants to the restroom.\nProvide continence care.\nAssist with changing clothing or protective garments when necessary.\nMaintain participant privacy and dignity.\nFollow infection-control procedures.\nObserve and report unusual urine, bowel patterns, diarrhea, constipation, bleeding, pain, or other concerns.\nDocument toileting assistance when required.\n\n Skin Care and Positioning\n\nThe CNA shall:\n\nObserve exposed skin during routine personal care.\nAssist with positioning and repositioning as directed.\nKeep participants clean and dry.\nImmediately report redness, bruising, swelling, wounds, pressure areas, skin tears, or other skin concerns to licensed nursing personnel.\nFollow established skin-care instructions.\n\nThe CNA shall not independently diagnose or treat wounds unless authorized, trained, and acting within the individual's lawful scope of practice.\n\n Activities and Social Engagement\n\nThe CNA shall:\n\nEncourage participant involvement in scheduled activities.\nAssist participants traveling to and from activity areas.\nAssist activity staff as needed.\nProvide individual support to participants requiring additional assistance.\nEncourage appropriate social interaction.\nPromote participant choice and independence.\nObserve participant responses to activities.\nReport significant changes in participation or behavior.\n\n Medication Safety\n\nThe CNA shall follow the center's medication-management policies.\nThe CNA shall:\n\nImmediately report medication concerns to licensed nursing personnel.\nNever independently change a participant's medication.\nNever borrow or share participant medications.\nNever conceal a medication error.\nImmediately report suspected medication errors or adverse reactions.\n\nMedication administration or assistance may only be performed when specifically permitted by applicable law, the CNA's credentials and training, and center policy.\nWhen uncertain, the CNA must obtain direction from the RN before performing a medication-related task.\n\n Infection Prevention and Control\n\nThe CNA shall consistently follow infection-control practices, including:\n\nHand hygiene.\nStandard precautions.\nAppropriate use of gloves and other PPE.\nBloodborne-pathogen precautions.\nSafe handling of bodily fluids.\nProper disposal of contaminated materials.\nCleaning and disinfecting participant-care equipment as assigned.\nFollowing isolation or infection-control instructions provided by licensed nursing personnel.\nReporting exposure incidents promptly.\n\n Participant Rights\n Documentation\n\nThe CNA shall accurately complete assigned documentation, which may include:\n\nADL assistance.\nToileting assistance.\nFood and fluid intake.\nVital signs.\nWeight.\nMobility assistance.\nActivity participation.\nBehavioral observations.\nChanges in condition.\nSkin observations.\nIncidents and accidents.\nDaily service documentation.\nOther documentation required by the Individual Plan of Care or center policy.\n\nDocumentation must be factual, timely, accurate, complete, and legible.\nThe CNA shall never falsify, improperly alter, backdate, or intentionally omit required documentation.\n\n Emergency Responsibilities\n\nThe CNA shall:\n\nRecognize and promptly report medical emergencies.\nImmediately obtain assistance from the RN or other appropriate personnel.\nCall 911 when directed or when emergency circumstances require immediate emergency assistance.\nPerform CPR and First Aid when trained, certified, and indicated.\nFollow emergency-response procedures.\nAssist with fire, severe-weather, disaster, and evacuation procedures.\nAssist ambulatory and non-ambulatory participants during evacuation according to training and assigned responsibilities.\nParticipate in required emergency drills.\n\n Transportation and Community Activities\n\nWhen assigned, the CNA may:\n\nAssist participants entering and exiting vehicles.\nAssist with wheelchairs and mobility devices.\nEnsure participants are safely seated.\nAssist with participant counts before departure and after arrival.\nSupervise participants during community activities.\nFollow participant-specific safety precautions.\nImmediately report transportation or community-outing incidents.\n\nParticipants shall never be left unattended in a vehicle when supervision is required.\n\n Communication and Teamwork\n\nThe CNA shall:\n\nCommunicate respectfully with participants, coworkers, supervisors, nurses, families, and other professionals.\nPromptly communicate participant concerns to licensed nursing personnel.\nParticipate in shift reports and care coordination.\nAttend required staff meetings.\nFollow supervisory instructions.\nWork cooperatively with DSP/Direct Care Staff.\nMaintain appropriate professional boundaries.\nNotify management promptly regarding absences or delays.\nAssist in maintaining safe participant coverage.\n\nMINIMUM QUALIFICATIONS\nThe CNA must:\n\nMaintain current certification/registry status as required for the position.\nMeet applicable Georgia Nurse Aide Registry requirements.\nBe qualified by education, training, competency, and experience to perform assigned duties.\nSuccessfully complete required background and registry screenings.\nMeet applicable pre-employment health requirements.\nComplete required orientation.\nComplete required ongoing staff training.\nDemonstrate competency in assigned participant-care duties.\nBe able to communicate effectively with participants and staff.\nBe able to read, understand, and follow participant care instructions and emergency procedures.\n\nPreferred Qualifications\n\nHigh school diploma or GED.\nPrevious adult day health, geriatric, long-term care, home health, assisted living, developmental-disability, or direct-care experience.\nExperience assisting individuals with mobility limitations, dementia, chronic health conditions, or disabilities.\nCurrent CPR and First Aid certification.\n\nPHYSICAL REQUIREMENTS\nThe CNA position requires the physical ability to safely provide direct personal care and participant assistance throughout the workday, with or without reasonable accommodation.\nThe CNA must be able to:\n\nStand and walk for extended periods.\nSit, bend, stoop, squat, kneel, reach, turn, and perform repetitive movements.\nAssist participants with toileting, grooming, dressing, hygiene, feeding, and other personal-care activities.\nAssist participants with walking, transfers, positioning, and mobility.\nPush, pull, and maneuver occupied and unoccupied wheelchairs and other mobility equipment.\nAssist participants entering and exiting vehicles when assigned.\nUse hands and fingers effectively for participant care and documentation.\nRespond promptly to falls, medical emergencies, fire alarms, and evacuation situations.\nPerform CPR and First Aid when trained and required.\nHear participant requests, alarms, emergency announcements, and safety-related communications.\nObserve changes in participant appearance, skin condition, mobility, behavior, breathing, and functioning.\nAssist with emergency evacuation according to the center's emergency plan and assigned responsibilities.\nWork in an environment where exposure to blood, bodily fluids, infectious materials, cleaning products, and other conditions associated with direct personal care may occur.\nWear required PPE and follow infection-control procedures.\n\nATTENDANCE AND RELIABILITY\nBecause the CNA provides direct participant care and may be included in required direct-care staffing coverage, dependable attendance is an essential job responsibility.\nThe CNA must:\n\nReport to work as scheduled.\nArrive on time.\nRemain at the assigned work area unless properly relieved.\nFollow established call-out procedures.\nNotify management promptly of absences or delays.\nNever abandon assigned participants.\nObtain authorization before leaving the center during an assigned shift.\nCooperate with staffing assignments necessary to maintain participant safety.\n\nCONFIDENTIALITY\nThe CNA must protect all participant medical, personal, behavioral, financial, and family information.\nParticipant information shall only be accessed, discussed, or disclosed when necessary for authorized job responsibilities and as permitted by center policy and applicable law.\nConfidentiality requirements continue after employment ends.\nPERFORMANCE EXPECTATIONS\nSuccessful performance includes:\n\nProviding safe and respectful personal care.\nFollowing the participant's Individual Plan of Care.\nFollowing RN/LPN instructions.\nPromptly reporting changes in condition.\nMaintaining participant dignity and privacy.\nCompleting documentation accurately.\nFollowing infection-control practices.\nMaintaining participant confidentiality.\nDemonstrating dependable attendance.\nMaintaining required CNA credentials.\nCompleting required training.\nWorking cooperatively with the
