Job Description

This full-time, 13-week contract opportunity is ideal for an experienced RN who thrives in a fast-paced, independent environment serving individuals in their homes. Youll use your clinical expertise to ensure continuity of care and optimal patient outcomes, while having the support of a dedicated healthcare team.

Schedule

Monday Friday, 8:00 AM 5:00 PM

Key Responsibilities

• Complete comprehensive OASIS assessments and create individualized care plans.
• Deliver skilled nursing care such as wound management, IV therapy, medication administration, and chronic disease intervention.
• Monitor patient progress, recognize changes in condition, and promptly update healthcare team and physicians.
• Educate patients and families on disease processes, medications, and preventive health strategies.
• Accurately document all assessments and care provided in Home Care Home Base (HCHB).
• Partner with therapists, social workers, and aides to coordinate multidisciplinary care.
• Attend case conferences, participate in staff meetings, and pursue continuing education as required.
• Ensure compliance with all regulatory standards and agency protocols.

Qualifications

• Active RN license in good standing in the state of practice.
• Minimum one year of recent home health nursing experience (field-based preferred).
• Proficient with Home Care Home Base (HCHB) and experienced with OASIS.
• Reliable vehicle, valid drivers license, and current auto insurance.
• Strong communication, critical thinking, and time management abilities.

$2,057 - $2,176/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
