About the Role:
Coney Island Homecare is looking for a compassionate and dedicated Hospice Aide to join our caring team in Bozeman, MT. In this meaningful role, you'll provide essential hands-on support to patients and families during life's most important moments. Responsibilities:

• Provide personal care assistance including bathing, grooming, dressing, and oral hygiene for hospice patients
• Assist with mobility, transfers, and repositioning to ensure patient comfort and safety
• Monitor and report changes in patient condition to the supervising nurse or care team
• Offer emotional support and companionship to patients and their families
• Assist with light housekeeping, meal preparation, and maintaining a clean, safe environment
• Document care activities accurately and in accordance with hospice care plans
• Collaborate with interdisciplinary hospice team to deliver coordinated, patient-centered end-of-life care

Requirements:

• Current Certified Nursing Assistant (CNA) or Home Health Aide (HHA) certification preferred
• Prior experience in hospice, palliative care, or home health care strongly preferred
• Ability to provide compassionate, dignified care to terminally ill patients
• Strong communication skills and ability to work within a multidisciplinary care team
• Valid driver's license and reliable transportation for home visits in the Bozeman, MT area
• Ability to perform physical tasks including lifting, bending, and assisting with patient transfers
• Empathetic, patient, and emotionally resilient with a commitment to end-of-life care excellence
