Job Summary

Always Best Care of Greater Cleveland, a leading provider of senior care services, is seeking a compassionate and skilled Licensed Practical Nurse (LPN) to join our Home Health team in Lorain County As a Licensed Practical Nurse (LPN) with our organization, you will have the opportunity to make a meaningful difference in the lives of our clients while working in a dynamic and rewarding environment.

Responsibilities
  • Provide high-quality, patient-centered care to clients in their homes, as part of our skilled home health care services
  • Conduct patient assessments, and implement interventions as needed
  • Collaborate with other healthcare professionals to ensure comprehensive and coordinated care
  • Perform wound care, and provide other skilled nursing interventions as ordered by physicians
  • Document patient care and progress accurately and thoroughly
  • Communicate effectively with patients, families, and healthcare team members to ensure seamless care transitions
About the Role

As a Licensed Practical Nurse (LPN) with Always Best Care of Greater Cleveland, you will have the opportunity to work in a fast-paced and dynamic environment, where no two days are ever the same. Our team is committed to providing exceptional care to our clients, and we are seeking a dedicated and compassionate LPN to join our ranks.

Requirements
  • Current LPN license in the state of Ohio
  • Strong communication and interpersonal skills
  • Ability to work collaboratively as part of a healthcare team
  • Physical ability to perform job duties, including standing, walking, and lifting
