Dentist Opportunity

Incentive options: $10,000 sign-on bonus plus $10,000 relocation bonus OR $120,000 sign-on bonus**

**$120,000 sign-on bonus requirements

To qualify for the $120,000 sign-on bonus, the provider must:

• Be employed full-time, consisting of 36 patient-facing hours and 4 administrative hours per week.
• Commit to working full-time, in-person for five (5) years at the Promise facility in Sioux Center, Iowa.
• Be new to full-time employment in a rural Iowa community.
• Acknowledge that the incentive payment:
• Agree to repay the full $120,000 bonus if any terms of the agreement are not fulfilled, including maintaining full-time employment for the required five-year period.
• Employees may reside outside of Iowa and commute, provided they work full-time and in person at a Promise facility.
