Care Coordination Specialist

Care Coordination Specialist Perform selected services and functions related to Transitions of Care & Utilization Management, including insurance authorization, payer communications, appeals and denial information, and scheduling. Required Qualifications High School diploma, GED, or equivalent program. Effective written, oral, and interpersonal communication skills. Proficient computer, word‑processing and Excel spreadsheet skills. Ability to learn new computer systems, e.g. EPIC and INDICIA. Knowledge of the unique clinical and discharge needs of patients. Strong time‑management and priority‑setting skills. Ability to work independently and as a team member. Self‑directed with the ability to take initiative to solve problems. Proficient computer skills including Microsoft Office and/or Google Suite. Preferred Qualifications Health Care Experience. #J-18808-Ljbffr Hackensack Meridian Health Inc.
