Case Manager (LVN)

Case Manager (LVN) Job Summary We are seeking a compassionate and skilled Case Manager (LVN) to join our team at Astoria Healthcare Center in Sylmar, CA. As a Case Manager (LVN), you will deliver efficient and effective nursing care while achieving positive clinical outcomes and patient/family satisfaction. Under the direction of a Registered Nurse (RN) Nursing Supervisor or Director of Nurses, you will operate within the scope of practice defined by the State Nurse Practice Act. Job Skills Comprehensive knowledge of nursing principles, including the ability to recognize and identify symptoms and manage emergency situations Knowledge of medications, their proper dosage, and expected results Ability to coordinate, delegate, and supervise unlicensed nursing functions Ability to create a patient-centered environment Basic knowledge of computer use Specific Educational/Vocational Requirements Graduate of an approved school for registered/practical or vocational nursing Must be currently licensed by the State Board of Nursing Experience preferred in rehabilitative or geriatric nursing Ability to read, write, speak, and understand the spoken English language Must provide verification of TST (tuberculin skin test) as required by state law and in accordance with Company policy Responsibilities/Accountabilities Patient Evaluation: Collects, reports, and documents objective and subjective data Observes conditions and reports changes in condition to RN Care Planning: Contributes to establishing individualized patient goals Assists in developing interventions to achieve goals Implements the plan of care Evaluates effectiveness of interventions to achieve patient goals and minimize re-hospitalizations Provision of Direct Patient Care: Reviews administered medications and orders as well as treatment ordered per physician orders Communicates pertinent data to RN and/or physician and/or managed care organization for approval of levels of care Documents accurately and thoroughly as needed Additional Responsibilities: Participates in Utilization Management or Interdisciplinary Care Management Meeting to facilitate appropriate discharge planning Communicates team's recommendations for discharge to the attending physician, patient, and family and managed care organization Assists with DME products needed for discharge and in-patient care Prepares patient/family for discharge including patient and family education Monitors patient care provided by unlicensed staff Performs other duties as requested Physical Capacities Ability to lift, push, or pull up to 50 pounds Ability to stand for long periods Ability to bend, stoop, or crouch to care for patients Ability to work in a fast-paced environment with frequent interruptions Benefits Competitive hourly rate of $30-$38 Opportunities for professional growth and development Collaborative and supportive work environment How to Apply If you are a motivated and compassionate LVN looking for a new challenge, please apply to this exciting opportunity at Astoria Healthcare Center.
