Excelcare at The Pines is seeking an experienced and compassionate Director of Social Services to lead our team in delivering highquality, personcentered support to the individuals and families we serve. This role is ideal for a strategic thinker who excels at building strong community partnerships, guiding multidisciplinary teams, and shaping programs that improve quality of life. - Assess the social, emotional, and psychological needs of residents and develop care plans.
- Provide counseling and support to residents and their families.
- Coordinate discharge planning and community resources.
- Maintain confidential and accurate resident records.
- Participate in interdisciplinary team meetings to ensure holistic care.
- Ensure compliance with state and federal regulations related to social services in a nursing home setting.
- Supervise social services staff and provide guidance and training as needed.
- Advocate for residents and address any concerns or issues they may have.
- Must be LSW or CSW in New Jersey
- Experience in a social services role in a healthcare or long-term care setting.
- Knowledge of state and federal regulations governing nursing homes.
- Strong communication, organizational, and interpersonal skills.
- Compassionate and dedicated to improving the quality of life for residents.
