A leading skilled nursing organization in the Cleveland region is seeking a highly professional Hospital Liaison to support strategic referral relationships and strengthen our presence across local hospitals and post-acute networks. This role is designed for an experienced healthcare professional who understands the Cleveland market and can represent the organization with clarity, confidence, and operational awareness.

Role Overview

The Hospital Liaison serves as the primary point of contact for hospital partners, ensuring efficient communication, timely assessments, and coordinated transitions of care. The position requires a polished, organized individual who can navigate complex hospital environments, maintain strong professional relationships, and support census development through consistent engagement.

Primary Responsibilities

• Establish and maintain productive relationships with hospital case managers, discharge planners, and clinical teams

• Conduct on-site evaluations and collaborate with hospital staff to facilitate appropriate admissions

• Communicate facility capabilities, service lines, and clinical strengths in a clear and professional manner

• Coordinate closely with internal Admissions, Nursing, and Therapy departments to ensure seamless transitions

• Monitor referral patterns, identify opportunities for growth, and provide feedback to leadership

• Represent the organization at hospital meetings, care conferences, and community healthcare events

Qualifications

• Experience in skilled nursing, post-acute care, or hospital liaison work

• Strong familiarity with Cleveland-area hospitals and referral processes

• Professional communication skills with the ability to engage clinical and administrative stakeholders

• Demonstrated ability to work independently, manage a territory, and prioritize multiple demands

• Reliable transportation for daily travel throughout the Cleveland market

Position Structure

• Full-Time, Monday–Friday

• Regional travel within Cleveland and surrounding hospital systems
