This position is responsible for supporting members in attendance, engaging and supervising associated programs and activities, and coordinating with internal and external care providers.At Community House, we value a strong work-life balance and aim to provide the best support for our workers. As a CHMHA employee, you will have access to a range of unique benefits, such as company-paid medical/dental/vision/disability insurance, a 403B retirement plan with employer match, continuing education credit, a personal unlimited Orca Card, and an employee assistance program with financial, mental, and legal assistance.RESPONSIBILITIESProvide group programming and activities that support opportunities for socialization and development of social and adaptive coping skills with members in cooperation with team members for all days Day Treatment is offeredCollaborate with care team to ensure all members access needed treatment, support, case management, and on-site appointments needed when attending Day Treatment programmingWork with members to engage in the pre-vocational program, including sign up, follow through, and compensation in accordance with agency and program policiesEnsure all interactions represent the five principles of trauma-informed care, including safety, choice, trustworthiness, collaboration, and empowermentSupervise medication windows to ensure the seven rights of medication use are followed: right person, right medication, right time, right dose, right route, right reason (including refusal), and right documentation according to agency provided medication trainingComplete daily documentation in electronic medical record on client interactions, medications dispensed when assigned, and other required documentation, ensuring an interaction and subsequent documentation are completed for all members across the teamPractice good financial stewardship of agency and contract resources, including how time is being spent, supplies are used, property is being maintained, etc.Monitor spending of Petty Cash and other agency money to ensure accountability and appropriate useMeet identified key performance indicators and metrics for the position as identified for the program, division, and agencyAssist with and support on-site cleaning, including skills training with members to maintain community spacesProvide immediate crisis intervention and resolution as neededAlways maintain client and staff confidentialityThe Day Treatment Specialist works with assigned staff to ensure high standards of care are provided within the Day Treatment program, including but not limited to:Providing appropriate levels of skills training and assistance are provided to clients in Activities of Daily Living (ADLs) and Instrumental Activities of Daily Living (IADLs) per their service plan, such as assistance in personal hygiene (prompting for showers, laundry, washing hands, etc.), cleaning, finances, transportation, etc.Assuring structured programming, including meal service program is available daily when Day Treatment is openPartnering as a team to deliver current programming and fill vacant position dutiesWorking collaboratively with larger outpatient team of providers to support client in actively working towards treatment goalsCoordinating care, such as scheduling, attending, and/or arranging transportation for appointments and other case management needsMaintaining and documenting in client charts, attendance trackers, etc. following agency, county, state, and federal guidelines.Providing input as part of client’s care team on service plan and progress, and collaborating with other providers such as primary care provider, case manager, etc.Facilitating regular group and individual activities, encounters, and specialized client events, such as outings and holiday gatheringsAssisting in admission and discharge planningMonitoring and managing day treatment milieuAssisting clients with access to medications through trained staffPerforming other duties as assignedBASIC QUALIFICATIONSExperience serving adults with severe and persistent mental illnessApply for Agency Affiliated Counselor credential with Washington Department of Health within 30 days of hire, if not otherwise licensed, and obtain and maintain registration and/or licensureSuccessfully pass criminal background check before hire dateObtain/maintain valid First Aid/CPR card or certificate within 30 days of hireHold a valid license for 1 year with clean driving record to use agency vehiclesHave legible handwriting and ability to do simple mathematicsAble to navigate and comprehend charts in electronic medical recordAble to work independently and within a teamAble to communicate information and ideas so others will understand.
