Group Home Supported Community Living Specialist

Description Full‑time position in Shenandoah, IA. Hours Monday‑Friday 6:30 am – 2:30 pm. The Supported Community Living Specialist (SCLS) reports directly to the assigned Team Leader and the Director of Residential Services. Key Duties and Responsibilities The SCLS teaches, trains, and assists individuals in developing skills for independent daily living, including health and medical care, community participation, and navigating public services. Coordinates and trains individuals to manage their own health and medical needs. Facilitates participation in community activities at least once per week when shifts coincide with day, evening, or weekend hours. Advocates for individuals' rights through participation in advocacy procedures. Prepares individualized diet plans to meet nutritional needs when required. Writes Individualized Intervention Plans for each served client. Provides emergency and safety skills training. Educates clients in managing personal finances, budgeting, and property use. Provides transportation to medical appointments, church, and other community sites. Transmits daily living skills training (e.g., meal preparation, laundry, housekeeping). Orients new clients in their homes, assesses skills, and supplies input to progress reports and Person‑Centered Plans (PCP). Monitors client goals, completes data collection, and documents progress. Supervises client finances, providing guidance on budgeting for personal needs and desires. Creates a supportive and therapeutic environment. Monitors use of personal property and assists with purchases when needed. Trains new staff on duties, schedules, agency policies, and state/federal regulations in coordination with the Team Leader and Director of Residential Services. Follows the assigned work schedule and never leaves 24‑hour care recipients unsupervised. Maintains accurate records and documentation using appropriate forms and procedures. Participates in staff meetings and scheduled in‑service training sessions. Ensures client residences meet cleanliness, safety, and health standards. Stays informed on programming, administrative updates, and changes by checking email and logs during each shift. Maintains strict confidentiality of all written and verbal client information. Documents behaviors and assists in preparing staff‑intervention plans as required. Helps manage physicians' ordered diets when necessary. Supports clients in activities of daily living such as meal prep, laundry, housekeeping, toileting, and bathing. Ensures client safety from environmental hazards. Essential Functions Provide quality care and transportation to clients. Train clients in daily living skills (e.g., meal preparation, laundry, cleaning, personal hygiene). Maintain housekeeping standards that meet client safety requirements. Work with clients who may exhibit verbal and physical aggression. Administer medication and maintain accurate records after training. Make immediate decisions when situations demand. Work according to assigned schedule, including overnight shifts where required. Complete written and electronic documentation as per agency requirements. Work independently, ensuring welfare and safety in a residential setting. Comply with HIPAA and other state/federal regulations concerning protected health information. Maintain a security clearance level appropriate to the role (Level 2 for direct‑care workers). Requirements Must be at least 18 years old with a high school diploma or GED. Prefer qualifications in teaching cooking, cleaning, and personal hygiene skills. Willing to work with individuals experiencing behavioral disorders that may involve physical aggression. Willing to clean biological fluids and waste. Able to work as a team member with good verbal and written communication skills. Self‑motivated and committed to client well‑being. Prohibited from having a conviction for assault, abuse, or bodily injury. Successfully complete pre‑employment physical exam, TB test, and drug screening. Maintain a valid Iowa driver’s license with acceptable driving record. Complete a 10‑hour medication‑management class. Post‑employment medical and drug screening every four years. Commit to confidentiality and cooperate with agency investigations. Physical Requirements Travel up to 70 miles between homes and offices. Enter and exit resident homes with variable accessibility. Walk up and down stairs while carrying laundry supplies. Work a full scheduled shift on assignment. Lift up to 50 pounds to perform job duties. Provide direct physical care for residents when necessary. Assist clients in transfers, including wheelchair to chair, bed, toilet, or bath. Assist clients in bathing or showering. Make multiple trips from vehicle to home carrying purchases such as groceries. Perform snow removal as needed for safety. #J-18808-Ljbffr
