Registered Nurse Case Manager (RNCM) – HospiceLocation: Cape Girardeau, MO Job Type: Full-Time | Salaried, ExemptLegacy Hospice is seeking a compassionate and skilled Registered Nurse Case Manager (RNCM) to join our mission-driven team. In this role, you'll deliver high-quality end-of-life care to patients in their homes while supporting families with empathy and respect. This is more than just a job — it's a calling to provide comfort, dignity, and support during life's most meaningful moments.What You'll DoPerform comprehensive patient assessments and create individualized care plansManage a caseload of hospice patients, coordinating care with the interdisciplinary teamAdminister medications and treatments per physician ordersEducate patients and families on symptom management, medication, and end-of-life careMonitor and update care plans based on ongoing patient assessmentsProvide emotional and clinical support to patients and caregiversCommunicate with physicians, care partners, and community resourcesMaintain accurate, timely clinical documentationParticipate in interdisciplinary team (IDT) meetings and on-call rotationSupervise ancillary staff and ensure continuity of careQualificationsRegistered Nurse (RN) licensed in the state of employmentBSN or Certified Hospice and Palliative Nurse (CHPN) preferredMinimum 1 year of nursing experience in hospice, home health, or a related fieldStrong interpersonal, clinical, and organizational skillsActive CPR/BLS certificationValid driver's license, auto insurance, and reliable transportationAbility to work independently and with a teamPlease note: Legacy Hospice is a drug-free workplace.
