Physical Therapist (Home Care) - Merrimack, NH

We are seeking an experienced Physical Therapist to provide high-quality, patient-centered care in a home health setting. This role focuses on helping patients restore function, improve mobility, and enhance overall quality of life through personalized therapy plans.

The Physical Therapist will collaborate closely with physicians and interdisciplinary team members while delivering care across a variety of residential settings.

Schedule: Monday - Friday (day shifts)

No regular on-call or weekend requirements

Shift differentials:

• 2nd shift: +$3/hour
• 3rd shift: +$6/hour
• Weekend: +$4/hour

Key Responsibilities:

• Develop and implement individualized therapy plans to restore strength, function, and mobility
• Evaluate patient progress and adjust treatment plans as needed
• Participate in discharge planning and interdisciplinary care conferences
• Travel within assigned territory to deliver in-home therapy services
• Supervise Physical Therapist Assistants (PTAs) as appropriate
• Attend staff meetings and required in-service trainings

Qualifications:

• Bachelor's degree in Physical Therapy
• Licensed/Registered Physical Therapist in the state of New Hampshire
• Minimum 2 years of experience in hospital or outpatient/clinic setting
• Current CPR certification
• Valid driver's license with satisfactory driving record
• Clean background check
