Every Tide-affiliated facility is guided by this purpose, from the front line to executive leadership.Job Description:Position SummaryThe Licensed Practical Nurse (LPN) provides direct nursing care to residents under the supervision of a Registered Nurse (RN) or physician. The LPN is responsible for administering medications, monitoring resident conditions, and ensuring quality care that promotes resident safety, comfort, and dignity.PAY RATE: $35-$40Essential Duties & ResponsibilitiesAdminister medications and treatments as ordered by the physicianMonitor residents’ conditions and report changes to RN or physicianPerform routine nursing care, including wound care and treatmentsAccurately document care, medications, and observationsSupervise and delegate tasks to CNAs and Nursing AssistantsAssist with admissions, discharges, and care planningRespond to medical emergencies and provide appropriate interventionsEnsure compliance with all state and federal regulationsMaintain infection control and safety standardsCommunicate effectively with residents, families, and healthcare teamProvide compassionate, resident-centered careQualificationsGraduate of an accredited practical nursing programCurrent, active LPN license in the state of employmentPrevious long-term care experience preferred but not requiredStrong clinical and critical thinking skillsCertifications (Required/Preferred)Active LPN licenseCPR certification (required or preferred per facility)Physical RequirementsAbility to stand, walk, bend, and lift for extended periodsAbility to lift/move residents (50+ lbs with assistance)Ability to respond quickly in emergency situationsWork EnvironmentLong-term care / skilled nursing facilityExposure to infectious diseases, bodily fluids, and medical conditionsMay require weekends, holidays, and various shifts (days, nights, weekends)Key SkillsMedication administration and clinical skillsLeadership and supervisionAttention to detailCommunication and teamworkTime management and organizationWork Location: In person #J-18808-Ljbffr The Gardens Health and Rehab
