Responsibilities Provide outpatient rehabilitation services in an in‑home setting for the older adult population. Supervise Occupational Therapy Assistants, temporary licensees, students and support personnel in accordance with state licensure requirements and professional standards. Maintain adherence to department and patient treatment schedules, timely clinical and billing documentation, and work‑related meetings. Attend and contribute to patient care, staffing conferences and other related meetings. Make referrals to other disciplines and agencies to address the comprehensive needs of the patient. Consult with rehabilitation team members, including community physicians, to select appropriate therapeutic regimens. Adapt programs and plans of care according to individual patient needs. Promote a safe environment and enforce elimination of fire and safety hazards. Order supplies and equipment as necessary. Clean, maintain, identify, and report equipment repair needs. Put patient service first: ensure patients and families receive high‑quality, compassionate service that recognizes individual needs and rights. Screen, examine, and evaluate patients, including history, systems review, and application of appropriate tests and measures; interpret findings to establish diagnosis, identify impairments, and design a treatment plan. Develop appropriate treatment goals and methods in collaboration with patient and caregivers; implement the occupational therapy treatment plan and complete all related documentation. Design, fabricate, fit, and/or apply adaptive devices; train patients in use of adaptive devices including orthotics and prosthetics. Select appropriate activities suited to the individual’s physical and mental capacity, intelligence level, and interest. Instruct patients in skills and techniques required for participation in functional activities; evaluate progress, attitudes, and behavior. Perform routine reexaminations as needed to modify or progress the plan of treatment. Perform other related duties as required. Services include but are not limited to application of therapeutic techniques in areas such as: cognitive skills muscle strength coordination endurance mobility perceptual abilities sensory awareness sitting and standing tolerance balance activities of daily living joint protection work simplification orientation physical agent modalities low vision falls risk management medication management health and wellness community integration and reintegration case management skills in home and community space Qualifications Bachelor’s degree in an occupational therapy curriculum accredited by ACOTE standards. Initial registration from the National Board for Certification in Occupational Therapy. Valid and unencumbered occupational therapy license required in the state of practice. Valid driver’s license or reliable, independent transportation to and from patient residences. Salary Posted Salary Range: USD $43.00 – USD $63.00 per hour. #J-18808-Ljbffr
