We are seeking an experienced Registered Nurse Case Manager to join a collaborative healthcare team. In this role, you will coordinate patient care, facilitate discharge planning, connect patients and families with appropriate resources, and work closely with interdisciplinary teams to ensure high-quality patient outcomes.

Schedule

• Day Shift
• Monday–Friday with rotating weekends
• Every other Saturday (possibly every 3rd Saturday once fully staffed)
• Shift options include:
• • 8:00 AM – 4:30 PM
  • 9:00 AM – 5:30 PM
  • 10:00 AM – 6:30 PM
  • 11:00 AM – 7:30 PM

Compensation & Incentives

• Starting Salary: $36.96/hour
• Mid Range (5–6 Years Experience): $40.48/hour
• Maximum (10–17 Years Experience): $44.00/hour
• Weekend Differential: $1.50/hour
• Sign-On Bonus: Available on a case-by-case basis
• Relocation Assistance: Available on a case-by-case basis

Key Responsibilities

• Guide patients and families through their care journey, including program orientation and discharge planning.
• Perform psychosocial assessments and develop individualized treatment and transition plans.
• Educate patients, caregivers, and families regarding treatment goals, available resources, and care plans.
• Collaborate with physicians, nursing staff, case management leadership, and interdisciplinary teams to ensure seamless, patient-centered care.

Qualifications

• Active RN or APRN license required.
• Associate Degree in Nursing or RN Diploma required.
• Minimum 2 years of recent acute care Case Management RN experience within the last year.
• Strong communication, organizational, discharge planning, and care coordination skills.
