Nurse Practitioner (NP) – Traveling Initial & Follow-Up Assessments Location: Connecticut (Travel to Multiple Facilities) Job Type: Part-Time / Flexible Schedule Specialty: Geriatrics / Long-Term Care Compensation: Competitive, based on experience Job Description We are seeking an experienced Nurse Practitioner (NP) to perform initial patient assessments and follow-up visits across multiple skilled nursing and long-term care facilities throughout Connecticut. This role offers flexibility, strong clinical support, and minimal documentation burden with a dedicated medical scribe assigned to assist with charting. The NP will travel to designated facilities over the course of the month to complete comprehensive assessments, establish care plans, and perform medically necessary follow-ups.
