About the Position

Job Title: Registered Nurse (RN) Profession: Nursing Specialty: Home Health Duration: 13 weeks Shift: Monday-Friday Hours per Shift: 8:00 AM - 5:30 PM or 8:00 AM - 5:00 PM Experience: Home Health experience required; strong Med Surg experience with some Home Health experience considered License: Valid nursing license required Certifications: Active BLS or willingness to obtain required Must-Have: COVID and FLU vaccination required Description: Prescribes, coordinates, and delivers care through the nursing process. Maintains the standards of professional nursing. Supervises CNA and coordinates care with other healthcare workers as assigned. Provides comprehensive initial assessments to all clients. Continually assesses physical, psychosocial, environmental, and community resources available, utilizing the principles of growth and development over the lifespan. Initiates and reevaluates the Plan of Care. Modifies Plan of Care as ordered or needed. Establishes discharge plans with patients, physicians, other disciplines, and caregivers. Completes OASIS data. Provides patient care using current Federal, State, and Agency regulations. Assesses educational needs of patients, families, and caregivers and implements appropriate teaching strategies. Completes documentation according to regulatory guidelines. Promotes a clean and safe environment of care, utilizing SAFE error prevention habits. Provides the highest standard of privacy and confidentiality in matters involving patients and coworkers by abiding by the Standards of Conduct.
