Location Requirements To qualify for this position you must live within a one hour drive of Hermitage, TN. About the Role AdvanceCare Health Services, LLC is seeking a dedicated Medical Coordinator with DDA/ECF/CLS experience to help us with our Medical Department. The ideal candidate will be responsible for managing administrative tasks and assisting in the smooth operation of the department. This is a hybrid role, combining work‑from‑home flexibility with some on‑site delivery responsibilities once weekly, and the possibility to work from home up to 4 days a week after initial training. You will need a 27\" or larger monitor, keyboard and mouse to use with our company‑issued laptop. If you come to the office to pick up your equipment, we can loan you the monitors, keyboard and mouse. Applicants must have high‑speed internet. You must be flexible and willing to work and learn on the fly! Duties Function as a medical case manager for the assigned individuals served in our Family Model Program Monitor the individual’s service plan in accordance with the regulatory guidelines Coordinate and arrange appointments, meetings, and other administrative matters on behalf of individuals served or staff members Assist in a supportive role for the individual served and his/her family Work with other Agencies on behalf of the assigned individual served Assist Director in interviewing prospective individuals served and their families Utilize computerized systems to schedule appointments and maintain patient records Perform administrative duties such as filing, data entry, and office management Utilize medical terminology accurately in communication and documentation Work with the Director in promoting community acceptance and support through participation in community activities and/or liaison efforts with other agencies and/or entities Assist in the performance of other duties as required by the needs of individuals served and as assigned by the Director Perform other duties and responsibilities as assigned and as needed Qualifications Cooperates with anyone who evaluates the home or represents the various agencies that place individuals in the home Ability to prepare necessary reports and records of individuals’ program progress with neatness, accuracy, and completeness Knowledge of practices and techniques of programming for individuals with intellectual disabilities and ability to exercise judgment in interpreting and implementing programs Ability to implement proper practices and techniques of supervision Ability to maintain effective verbal and written communication with staff, individuals’ families, individuals and outside service providers Must be flexible and organized Strong administrative skills with attention to detail Ability to multitask and prioritize tasks effectively Must maintain liability insurance, good driving record, and reliable transportation Must be able to work without close supervision The ability to interact with a wide range of people Good computer skills: MS Word, Excel, Outlook, Google Docs, and Windows Work Environment This full‑time, 40‑hour‑per‑week role offers a hybrid work arrangement, using company‑provided systems to stay connected and productive. It requires a quiet, professional workspace when working from home, reliable internet, and the ability to stay focused and organized without direct supervision. Regular communication with leadership and team members is essential, and when working from home, timely responsiveness during business hours is expected.
