Licensed Practical Nurse (LPN) As a Licensed Practical Nurse (LPN) on our team, you will play a vital role in delivering high-quality patient care to our residents. We are seeking a compassionate and skilled nurse to provide medical and rehabilitation services to our short-term and long-term care residents. Licensed Practical Nurse (LPN) Responsibilities: Provide direct patient care to residents, including administering medications, wound care, and vital sign monitoring Develop and implement individualized care plans in collaboration with the healthcare team Document resident progress, report any changes or concerns to the healthcare team, and participate in interdisciplinary care conferences Collaborate with other healthcare professionals to ensure seamless transitions and optimal outcomes Maintain accurate and up-to-date medical records Licensed Practical Nurse (LPN) Requirements: Current LPN licensure 0-2 years of experience in a post-acute care setting (new graduates welcome to apply) Strong communication and interpersonal skills Ability to work collaboratively as part of a team Basic computer skills and familiarity with electronic medical records Licensed Practical Nurse (LPN) Benefits: 401K Shift differential Health, Dental, Vision Competitive wages based on experience #J-18808-Ljbffr WF TPS
