Position Overview: Under general direction, the Patient Navigator Supervisor provides operational and functional oversight for Patient Navigator services across Harris County Public Health clinical sites and mobile units. This position standardizes workflows, ensures documentation quality and service fidelity, coaches and develops staff, monitors performance metrics, and strengthens coordination with clinic teams, co-located programs, and community partners. The Supervisor serves as the program lead for patient navigation operations, promoting timely screening, warm handoffs, closed-loop referrals, and effective resolution of non-medical barriers that affect access to care and continuity of services. Duties & Responsibilities: Supervise, coach, and support Patient Navigators across assigned sites; conduct regular one-on-one check-ins, case reviews, and team meetings to reinforce expectations, build skills, and address performance issues. Standardize and monitor navigation workflows, including patient identification, screening processes, referral pathways, warm handoffs, closed-loop follow-up, and documentation requirements across fixed and mobile sites. Strengthen and maintain the Community Resource Directory and referral relationships by coordinating with internal programs, HPCC partners, and external community organizations to improve service access and referral quality. Review charts, notes, and work queues to ensure documentation quality, timely follow-up, and compliance with program standards, privacy requirements, and operational protocols. Monitor and analyze program performance indicators, including screening completion, referral volume, warm handoff rates, service connection outcomes, timeliness, and documentation compliance; prepare summaries and reports for leadership. Serve as the escalation point for complex cases, unresolved barriers, cross-site workload balancing, and coordination issues involving clinic operations, co-located programs, or community partners. Requirements Education: Bachelor's degree in Social Work, Psychology, Public Health, Health Administration, or related field. Experience: At least two (2) years of experience in patient navigation, case coordination, community health, care management, or a related health or human services setting. At least one (1) year of lead, supervisory, or program coordination experience. Experience working with undeserved, low-income, culturally diverse populations. Licenses/Certifications: Valid Texas Driver's License. Knowledge, Skills and Abilities: Knowledge of patient navigation, care coordination, community resource linkage, and social drivers of health workflows in a clinical or public health setting. Ability to supervise, coach, and develop staff while maintaining accountability for quality, timeliness, and service standards. Ability to analyze workflow, monitor performance data, identify gaps, and implement process improvements across multiple service locations. Strong interpersonal, written, and verbal communication skills, including the ability to collaborate effectively with providers, clinic staff, leadership, and community partners. Ability to manage sensitive situations, exercise sound judgment, and handle safety, clinical, or operational issues appropriately. Working knowledge of health record documentation practices and Microsoft Office applications. Community Partnership Skills: This competency focuses on collaborating with communities to address the systems and policies affecting health, fostering engagement, shared ownership, and sustainable improvements in well-being. Public Health Science Skills: This competency focuses on using public health science and evidence-based approaches to develop and improve policies and programs, ensuring effective delivery of the 10 Essential Public Health Services. Management and Finance Skills: This competency focuses on managing human and financial resources, promoting equity and collaboration, and optimizing organizational performance to support effective public health programs. Leadership and Systems Thinking Skills: This competency focuses on aligning stakeholders, fostering innovation, and managing change to implement effective public health initiatives that strengthen community health. Preferences Education: Master's degree in Social Work, Psychology, Public Health, Health Administration, or related field. Experience: -Experience using electronic health record (EHR) system. Language: -Bilingual English/Spanish. General Information Position Type and Typical Hours of Work: This is a full time position, 40 hours per week. May be required to work outside normal business hours as operational needs require. Travel across Harris County sites is required to support staff, operations, training, meetings, and workflow monitoring. Work Environment: This position operates in community clinic, mobile, office, and occasional field settings and routinely uses standard office equipment. Physical Demands: Frequently required to sit, stand, bend, communicate, and use manual dexterity. May occasionally lift or move up to 20 pounds.
