Description

• Conduct evaluations of patients in either inpatient or outpatient environments and develop personalized treatment plans
• Deliver physical therapy interventions aimed at enhancing mobility, strength, balance, and functional capabilities
• Work collaboratively with nurses, physicians, case managers, and other rehabilitation professionals to optimize patient outcomes
• Educate patients and their families on therapeutic objectives, discharge processes, and home exercise regimens
• Keep detailed records of evaluations, treatments, and patient progress in line with facility and regulatory standards
• Engage in care team discussions and interdisciplinary conferences to guarantee coordinated patient care

Education

• Doctorate in Physical Therapy (DPT) or an equivalent qualification from an accredited institution

Certifications

• State licensure as a Physical Therapist
• CPR certification preferred
• Additional specialized certifications (e.g., neurological, geriatric) are beneficial but not mandatory

Skills

• Prior experience is highly desirable
• Capable of treating patients with complex medical and mobility challenges
• Excellent communication and teamwork abilities
• Proficient in digital charting and clinical documentation systems
