POSITIONS: Worksite locations in Crane, Pecos and Balmorhea Texas.

POSITION SUMMARY: The Surveyor/Implementor reports to the Site Manager where applicable and otherwise to the Executive Director and Managing Director of Healthy Communities – Permian Basin. Texas Tech University Health Sciences Center, in collaboration with The Beacon Alliance and the Permian Strategic Partnership, is leading a population health initiative to improve the health and well-being of rural communities across the Permian Basin (West Texas and Southeast New Mexico).

The Healthy Communities – Permian Basin program seeks to assess the prevalence of undiagnosed and undertreated health conditions in rural areas and deliver innovative, person-centered solutions to improve community health outcomes. There are currently program sites in Texas in Crane, Balmorhea, Saragossa, and Pecos. A site in Hobbs, NM will be opening Fall 2026.

As part of the Healthy Communities – Permian Basin team, the Surveyor/Implementor plays a key role in bridging the gap between healthcare systems and rural communities. This position supports the collection of community-level data and biomarkers to identify health needs and inform targeted interventions. The Surveyor/Implementor collaborates closely with interdisciplinary team members to advance program objectives.

The ideal candidate will spend the majority of their time in the field, engaging directly with community members. This includes program activities in community facilities, homes, and churches. The role requires a flexible schedule, including evenings and weekends, to attend community events and meet with individuals based on their availability.

Key Responsibilities

The Surveyor/Implementor will:

  • Conduct outreach activities to promote health awareness and preventive care.
  • Carry out outreach and engagement at community facilities, community events, and door-to-door household.
  • Provide culturally appropriate health education and informational resources, demonstrating respect for the unique values and needs of rural communities.
  • Perform basic health screenings, including measuring blood pressure and weight, conducting finger-prick and HbA1c testing, and installing Continuous Glucose Monitors (CGMs).
  • Encourage individual and community participation in health assessments and educational initiatives to improve overall well-being.
  • Identify health-related concerns, collect relevant data, and report findings to Healthy Communities – Permian Basin program leadership.
  • Complete all required administrative and technical training prior to fieldwork.
  • Demonstrate proficiency in survey administration and data collection activities.
  • Identify eligible participants within selected households and administer questionnaires accordingly.
  • Manage and maintain field technology, including tablets and Wi-Fi hotspots.
  • Assist community members with setting up and using health-related mobile applications.
  • Track survey completion and biomarker data to support follow-up activities.
  • Work in non-traditional environments (e.g., outdoors, private homes, mobile units).
  • Participate in team debrief sessions and contribute to continuous program improvement.

Qualifications

Education

  • High school diploma or equivalent required.
  • Enrollment in or completion of an advanced degree program in a health-related field (e.g., nursing, physician assistant studies, pre-med, psychology, public health, or social work) strongly preferred.

Communication & Interpersonal Skills

  • Strong communication and relationship-building skills.
  • Bilingual in English and Spanish preferred.
  • Demonstrated cultural sensitivity and emotional maturity when working with diverse populations.
  • Ability to effectively deliver health-related information to individuals and groups.
  • Proven ability to work collaboratively within a team environment.

Additional Requirements

  • Must be at least 18 years of age.
  • Basic knowledge of diabetes and hypertension.
  • Strong organizational and time management skills.
  • Demonstrated interest in improving health outcomes in rural communities within the United States.
  • Availability to work a flexible schedule, including evenings and weekends.
  • Valid driver’s license and current automobile insurance required.

Supervision Requirements: This position has no supervisory responsibilities.

Physical Requirements: This position requires the ability to work in field-based settings and travel to various community locations. The physical demands described are representative of those required to successfully perform the essential functions of this role. Duties may include standing, walking, climbing stairs or ramps, balancing, stooping, and handling materials, as well as frequent use of hands to operate a computer, tablet, or other field equipment. The Surveyor/Implementor must be able to lift and move items weighing up to fifty (50) pounds. This role requires driving to program-related locations and working in a variety of environments, including outdoor settings, where exposure to varying weather conditions, including heat, may occur.
