Physical Therapist (PT) - Outpatient

Clinician – Healthcare

Schedule: 8 HR Days, 8:00 AM - 4:00 PM, 40 hours a week, primarily Monday through Friday

Assignment Details: Guaranteed Hours: 40 per week, Contract Length: 13 weeks

Roles & Responsibilities:

• Provide high-quality patient care in accordance with facility protocols and guidelines.
• Conduct assessments and develop individualized care plans.
• Collaborate with interdisciplinary teams to optimize patient outcomes.
• Educate patients and families on health management and preventative care.
• Maintain accurate and timely documentation of patient progress.
• Participate in continuous quality improvement initiatives.
