Application Details Requisition #: View phone number on click.appcast.io Shift: Day Status: Full Time with Benefits Responsibilities Provide professional nursing care to patients in their homes, assessing, planning, implementing, and evaluating individual care needs. Ensure timely compliance with physician orders and proper documentation of visit frequency and care progression. Follow policy, procedure, and standard of care for assessment, planning, intervention, and evaluation of patient care; demonstrate holistic care for patients and significant others. Perform treatments and procedures per established standards, document all activities, and collaborate with interdisciplinary team. Apply professional clinical judgment and critical thinking, documenting decisions through appropriate systems. Identify payor requirements, maintain authorizations, and use secure messaging for insurance nurse confirmations. Complete nursing assessments, medication reconciliation, and collaborate with physicians on plans of care; update care plans within two weeks of patient status change. Implement teaching strategies to meet established goals and document them in the medical chart. Report patient care problems to managers or appropriate personnel in a timely manner with solutions. Liaise among physicians, managers, and care team to coordinate services effectively. Communicate patient information during transfers of care loads on a daily or scheduled basis. Complete routine assignments accurately and independently; meet productivity requirements. Serve as clinical supervisor of home health aide staff, conduct supervisory visits, and evaluate aide performance. Stay current with updated standards and incorporate them into practice; document knowledge of current guidelines. Perform other duties and projects as assigned and demonstrate flexibility. Adhere to HMH organizational competencies and standards of behavior. Lift a minimum of 50 lbs., push and pull a minimum of 50 lbs., and stand a minimum of 8 hours a day. Required Qualifications Associate’s Degree in Nursing or Diploma Certificate in Nursing. At least six (6) months RN experience or at least one (1) year LPN experience, preferably in home care or hospice. Preferred Qualifications BSN degree. Licenses & Certifications NJ State Professional Registered Nurse License. AHA Basic Health Care Life Support HCP Certification. Compensation Minimum rate of $90,272.00 Annually. Additional compensation may vary based on market data, experience, education, certifications, skills, location, internal equity, budget, and funding.
