Social Services Manager

Role: Manager, Social Services Lead community and social services programs within a healthcare network. Oversee a multidisciplinary team providing psychosocial assessments, care coordination, and discharge planning. Supervise staff, develop programs, and monitor quality and outcomes. Collaborate with medical providers and community agencies to improve patient care. Utilize clinical social work expertise and data-driven approaches to enhance access, equity, and health outcomes for diverse and vulnerable populations.
