RN Care Manager (PACE Program)

We are seeking a compassionate and experienced Registered Nurse (RN) Care Manager to join our team. In this role, you will coordinate and deliver high-quality care for older adults, working closely with an interdisciplinary care team to support participants’ health, independence, and overall well-being.

• Coordinate care with physicians, social workers, and interdisciplinary team members
• Perform assessments, triage, and ongoing monitoring of participants
• Develop and manage individualized care plans
• Provide skilled nursing care in clinic and home settings
• Educate participants, caregivers, and families
• Oversee medication management and chronic condition monitoring
• Ensure accurate documentation and regulatory compliance
• Support quality improvement and care coordination initiatives

• Active Registered Nurse (RN) license (NC)
• Graduate of an accredited nursing program (BSN preferred)
• Minimum 2 years RN experience
• Experience working with elderly or frail populations required
• Home care experience preferred
• CPR certification (or willingness to obtain)
• Valid driver’s license and reliable transportation

• Strong clinical judgment and critical thinking skills
• Excellent communication and teamwork abilities
• Ability to work independently and manage multiple priorities
