The Nurse RN/LPN provides skilled nursing care to geriatric patients in a long-term care and skilled care facility. Reporting to the Director of Nursing, this role supports day and night shift coverage while delivering safe, compassionate, and coordinated patient care.
Responsibilities
- Assess geriatric patients and monitor changes in condition.
- Develop and implement individualized care plans.
- Administer medications safely and accurately.
- Monitor and document vital signs.
- Provide wound care and related treatments.
- Maintain accurate clinical documentation.
- Educate patients on care needs and treatment plans.
- Coordinate care, support infection control, and assist with discharge planning.
Required Qualifications
- Active Registered Nurse (RN) license.
- Active Licensed Practical Nurse (LPN) license.
