Physical Therapist

Basic Qualifications Licensed in the state of practice and free from disciplinary action. Valid driver’s license and good driving record. Other Qualifications Clear understanding of therapy in the homecare setting. Ability to assess client needs and collaborate with individuals within and outside the company. Strong analytical, problem‑solving, communication, and leadership skills. Computer proficiency and ability to learn different computer programs. Physical Requirements Lift, push, and/or pull minimum 50 lbs. Extended periods of walking, standing, and driving. Significant potential for exposure to latex. TB testing per regulations. Responsibilities Evaluate and implement physical therapy program on referral of client’s physician, provide written evaluation and plan of care, and write progress notes and discharge summaries per departmental policy. Coordinate physical therapy with nursing staff and other rehabilitative services. Provide in‑services to administration, medical staff, other therapists and family members. Attend meetings as needed at the home health agency and mandatory in‑services. Attend required educational in‑services to meet license renewal requirements. Supervise PTA and delegate treatment plan. Employ and follow all appropriate agency infection‑control policies and procedures. Observe, record, and report to the physician the client’s reaction to treatment and changes in the client’s condition. Willingness to keep up with current physical therapy practices. Ability to exercise independent judgment. Willingness to participate in activities that promote professional growth and development.
