Licensed Practical Nurse (LPN) – Wound Care / Treatment Nurse

Lineville Health & Rehabilitation is seeking an LPN Wound Care / Treatment Nurse who wants to use their nursing skills in a specialized clinical position focused on healing, prevention, and resident outcomes.

If you're ready for a change from rotating shifts or a traditional floor assignment, this position offers the opportunity to develop your wound-care expertise while enjoying a consistent Monday–Friday day schedule.

You'll provide treatments, monitor skin and wound conditions, document progress, and work closely with RNs, physicians, CNAs, and the interdisciplinary team.

You won't just provide the treatment—you'll get to see the progress.

Position Overview

The LPN Wound Care / Treatment Nurse provides wound and skin treatments under nursing leadership and in accordance with physician orders, established protocols, facility policy, and the LPN scope of practice.

You'll become an important member of the clinical team supporting skin integrity, wound prevention, treatment, documentation, and resident comfort.

What You'll Do

• Provide wound and skin treatments according to physician orders and established protocols
• Monitor wounds and skin conditions and report changes appropriately
• Document treatments and resident response accurately and timely
• Participate in routine skin and wound rounds
• Help identify residents at risk for skin breakdown
• Support preventive interventions, including turning, repositioning, and pressure-relief measures
• Collaborate with RNs, physicians, and interdisciplinary team members regarding treatment plans
• Participate in care planning related to skin integrity
• Monitor treatment supplies and communicate supply needs
• Reinforce skin-integrity and wound-prevention practices with nursing staff and CNAs
• Support infection prevention and quality-improvement efforts related to wound care
• Promote resident comfort, dignity, healing, and quality of life

Qualifications

• Current Alabama Licensed Practical Nurse (LPN) license in good standing
• Strong nursing observation and documentation skills
• Excellent communication and organizational abilities
• Compassionate, resident-centered approach
• Ability to work effectively with nursing and interdisciplinary team members
• Long-term care, rehabilitation, acute-care, or geriatric nursing experience preferred
• Previous wound care or treatment nursing experience preferred
